COVID – 19 Measures Implemented

CARE FERTILITY IS OPEN FOR PATIENTS

As the Coronavirus and its impact continues to evolve, CARE Fertility remains focused on the health and safety of our
patients, employees, and community.
CARE Fertility is open during this unprecedented crisis and our doctors, nurses, and staff will continue to provide care to
our patients. We continue to treat existing patients and accept new patients. All services have been consolidated and
centralized to our Glendale office only. Our other locations will remain temporarily closed. Not to worry, our staff in
Glendale can arrange appointments with your doctor by phone or video conferencing. Additionally, We are ensuring
that patient appointments are staggered to minimize contact with other guests or patients during your visits.

NOW OFFERING PHONE & VIDEO CONFERENCING

We offer the use of telemedicine (phone or video conferencing) in lieu of an in-person visit. This is available to anyone
who wishes to schedule a consultation or new patient appointment.

MANDATORY FACE-COVERING

We take the health of our patients and employees seriously. ALL patients, staff, and visitors must wear a simple cloth
face covering or personal mask to enter our office. Please understand this policy is for your own protection, as well as
the protection of your fellow patients and our staff.

Effective April 10, 2020 – CARE Fertility Policy Updates:

● LA County requires all customers of essential businesses and employees to wear face coverings while inside
any business.
● All patients and partners who enter the office must wear a mask.
● All employees must wear a mask.

VISITORS ACCOMPANYING PATIENTS

To minimize risks to all patients and staff, all in-person diagnostic and monitoring appointments, including ultrasound
and blood work visits, and any procedures including egg retrievals, embryo transfers, inseminations, or surgical
procedures are limited to the patient only. Partners are able to accompany patients to the office, however we ask that
they wait in their vehicles during the visits or procedures for their safety and the safety of other patients and staff.

Effective May 4, 2020, CARE Fertility Policy Updates:

● Only patients with appointments will be permitted to enter the office.
● Any guests including children, family members, and friends will not be permitted to enter the office.
● Any guests including partners that accompany the patients to their visit, will be asked to wait in their vehicles
during the visit.

SCREENING AT CARE FERTILITY

Please be reassured that CARE Fertility performs ongoing screenings to each of our employees. We want to make sure
we have a healthy team taking care of our patients. To also ensure patients are ready to enter the clinic, we have
adjusted our policy and we are now administering temperature checks and health screenings upon check-in.

Effective May 4, 2020, CARE Fertility Policy Updates:

● Only patients with appointments will be permitted to enter the office.
● Each patient and their partner that enters CARE Fertility will have their temperature check upon entering
the office.
● Each patient and their partner that enters CARE Fertility will be provided hand sanitizer upon entry.

COVID-19 VACCINATION FOR PREGNANT AND LACTATING PATIENTS

ONGOING SAFETY RECOMMENDATIONS

  • Frequently wash hands with soap and water for at least 20 seconds
  • If unable to wash with soap and water, use an alcohol-based hand sanitizer
  • Avoid touching your face eyes or mouth with unwashed hands
  • Cover your mouth and nose with a tissue when coughing or sneezing. Throw the tissue away, and then wash or sanitize
    your hands. If a tissue is not available, cough or sneeze into your upper sleeve/arm
  • Avoid handshakes and close contact
  • Don’t share food/bottles
  • Clean your work area and high touch surface areas with disinfectant regularly
  • Avoid close contact with people who are sick
